[board] Reimbursement requests

In my role as Treasurer I want to continue to ensure that there is complete transparency in our record keeping.   Malcolm had developed the attacjed form which I want to continue to utilize when reimbursing members for expense incurred for the benefit of our Association.   
 
The form is straight forward and I do not believe this requires any Board action so this is primarily a FYI.  It might be good to have this posted our webpage for ease of members who are requesting reimbursement.
